Taking the first steps
People eligible for our weight management service will receive the support they need to lose weight and live a healthier life.
Individuals meeting the eligibility criteria for our weight management service will be provided with ongoing support and guidance to lead a healthier lifestyle. Upon embarking on your 12-week journey, you will undergo an initial consultation. Subsequently, you will then participate in two group sessions per week, where you will receive advice on nutrition and a circuit-based class.
Online there are so many ways to keep on top of your weight management; things like weight loss plans are all online! Here is an example.

Can my carer come with me?
Yes! Your carer is more than welcome to come with you to help you during your time with us.
Can I bring my guide dog?
Yes, guide dogs are welcome.
Where will my sessions take place?
We have venues across West Cheshire and can find a location that is suitable for you to attend.
How many times can I use your service?
You are able to come and see us once in a 12 month period.

Cheshire Change Hub services are only available to residents of Cheshire West and Chester, or those registered with GP’s in the area.
You may be able to find similar services in your own area, and we also have some general information which may be of interest to you.

Are you registered as having any of the following?
- Learning disability
- Severe mental illness (SMI) - PTSD, bipolar disorder, schizophrenia, psychosis
- Hypertension
- Physical disability (a person’s physical functioning is clinically evidenced as having a long-term negative effect on their ability to exercise, and they are in receipt of a qualifying disability benefit related to their physical function)

Can I refer myself?No, a GP needs to refer you to Cheshire Change Hub.
You will receive:
- 12-week FREE programme
- Group sessions
- Each session will include physical activity, educational content, body composition assessments, nutritional education, and goal-setting activities
- On completion of 12-week programme, you’ll be eligible for a discounted 12-month Brio membership at £22pm
